This checkout form must be completed by the lead artist only — the person who attended the walkthrough and made the booking. As the lead artist, you are solely responsible for opening and closing the space. This cannot be delegated.

All checklists are subject to staff review. Items marked complete will be verified.

To ensure the space stays safe and organized, if 3 checklist items are found incomplete over time, a walkthrough will be required before making your next booking. Access to the booking system will be temporarily paused until the walkthrough is completed.
